Neonatal Sepsis

1. Definition:
• A bacterial blood infection in newborns younger than 3 months; can be either early-onset (24hrs-1st week) or late-onset (newborns within 1 week-3 months).

2. Causes/Contributing/Risk Factors:
• Premature rupture of membranes
• Premature infants
• UTIs in mother
• Maternal GBS colonization
• Chorioamnionitis
• Low Apgar score
• Low Birth Weight
• Birth asphyxia
• Meconium staining 3. S/S (maternal & fetal):
• High fever
• N/V/D
• Respiratory distress
• Poor sucking/latching
• Seizures
• Lethargic
• Cold and clammy skin
• Reduction of body movements

4. Labs to Monitor:
• Ultrasound
• Urine culture
• CBC
• Lumbar puncture/Spinal tap

5. Treatments/Precautions:
• Medications to help with BP/Heart functions

Nursing Interventions: (10)
• Give IV fluids
• Monitor VS
• Assess skin for change in color
• Assess for Respiratory distress
• Provide comfort
• Assess Pain
• Monitor for side effects resulting from any medications
• Provide proper information to parents
• Monitor I&O
• Provide comfort and support if Spinal tap is ordered

7. Nanda Diagnoses: (5)
• Anxiety R/T health status change of newborn
• Infection R/T positive blood cultures
• Decreased Cardiac Output
• Risk for ineffective breathing pattern R/T respiratory distress
• Fluid Volume Deficit R/T imbalance of activity and food intake

8. Long Term Problems Associated With Condition/Complication:
• Delay in motor functions
• Developmental delay
• Cerebral palsy
• Seizure
• Possible hearing loss
