Patient’s falls are a well-known problem in healthcare facilities, and the consequences of falls can be detrimental to both the patient and the facilities. Throughout the years there have been several policies to help in preventing falls such as restraints, placing patients near the nursing stations, bed alarms, new beds that can be set within two inches from the floor. The incidence of falls continues to rise. The purpose of this discussion board is to discuss the implementation of intentional nursing rounding to prevent falls within our facility which is based on evidence-based nursing.

After comparing the data, the staff proceeded to research information on the subject of IR in the hospitals. While investigating several types of theories, the team decided to use the four P’s to effectively implement during IR to help facilitate the decrease in falls. The four P’s to consider are position(is the bed rail up, is the bed down, and does the patient need repositioning). Personal needs (does the patient need to go to the bathroom, do they want or need a snack or is there anything else they need). Pain (are they in pain, do they need pain medication) and placement ( is the call light, bed table, phone near them). Furthermore, a time clock was implemented which the staff would initial every hour that the four P’s were addressed and if not why? (Harris et al., 2017). After the implementation, the managers are required to daily rounds with the charge nurse of the floor every morning and go over the charting of the rounding and check to make sure it was completed and then evaluated the incidence report to verify if there were any fall for the previous day.
