Therapeutic Practitioner Formative Assessment In this essay, I will be examining the care given to one of the long-time patients that I met as a student nurse on an acute-care women’s ward. I will be focussing on the five pillars of therapeutic care: communication, partnership, safety and risk, pharmacology, and self-evaluation. Following the NMC code’s (2018) specifications of confidentiality, the patient’s name will remain private, and in this place she will be referred to as Judy. Judy is an 87-year-old woman presenting with general unwellness and right flank and leg pain and, due to her condition, was admitted as a P6 patient, meaning she was nursed in bed. Self-Evaluation Over the course of this essay, I have analysed the care I provided to a patient presenting with right flank and leg pain as well as general unwellness. Following NMC standards of care, I have kept the patient’s identity private, opting to refer to her as Judy. Judy underwent a complete right knee debridement and PT and OT over the course of a month while expressing extensive fears of walking and standing due to her history of falls. I worked closely with various teams, including PT and OT, the surgical team, and the pain team, to ensure that Judy was kept as comfortable as possible and that her recovery was on the right track. By working with other members of the nursing team, we were able to progress Judy’s recovery faster than expected, building her confidence in her ability to stand and support herself with a frame.
