Presenting Problem: Alysha is a 17 y/o AA female presenting with a dx of Bipolar disorder type 2, recent episode depressed. She is currently in NNBHC acute unit on a TDO due to feeling depressed and she tried to hang herself. Her father called the police and she placed a belt around her neck. When she saw the police she was tightening the belt and then the police attempted to take the belt off, she tried to fight them. The crisis services were called and the patient was taken to CHKD. The patient has a long history of psychiatric history; she has been treated at NNBHC x3 admissions, and last discharged from Kempsville Behavioral Center. Patient states her parents were separated divorced when she was 3 years old. She states her other had some emotional problem and had problems with alcohol, drugs and also was incarcerated in jail. She sates she has one biological sister, then she has a half-sister and half-brother. She grew up with her paternal aunt and sometimes she has been with her grandmother. She often gets in arguments at school. She gets into fights and at one time, she was sent into detention for that. Patient states she stayed there for a year and she had to repeat one grade at that time. Patient has history of sexual, physical and emotional abuse in the past. She states she smoked marijuana off and on, an denies drug use, and alcohol use. Verbal altercation between Alysha and her father happen often over discipline, which spirals to aggressive or suicidal behaviors. Father communicates that he has a difficulty with keeping her consistent with medication compliant. Alysha is in the 10th grade at Booker T Washington High School, with an IEP for
