Working in partnerships means that various health and social service professionals will work together in a team, as opposed to working as an individual. Many practitioners will be involved in Albert’s care plan, such as an endocrinologist, a dietician, a podiatrist and an eye doctor. Albert would have regular interaction with his endocrinologist who will monitor his diabetes, help him with any complications he may have, and answer and queries Albert may have. Albert will need to have various meeting with a dieticians in order to control his diabetes. For example, a diet plan will be created for Albert, which will ensure he meets his nutritional needs, as well as maintaining his blood sugar levels. This diet plan will also help Albert to either maintain his weight or to lose weight, to ensure his diabetes does not being uncontrollable. After losing his leg from an infection due to his diabetes, Albert visits a podiatrist to ensure the same fate doesn’t happen to his other foot. The podiatrist job is to inspect Albert’s feet, to prevent infections and to give advice on what Albert can do himself. Over time, high blood sugar can damage blood vessels in your eyes, which makes it vital to visit an eye doctor, at least once a year, in order to look for any problem. …show more content…
For example, everyone will know what the targets of the care plan are; timing will be discussed so visits do not overlap; everyone will meet together to review the care plan; and there will be a shared log to view what has been done and the progress that has been made. It will also mean that the whole process is easier for Albert as he wont have to co-ordinate between his practitioners and his appointments himself. It also means that Albert will receive the care and attention that he requires from each practitioner effectively, as the care he is required to receive has been drawn up in the care
