On 11/03/15 Ms. Maritza Torres came to the SSO to meet with HS De La Torre to develop a Housing Plan in order to explore possibilities for the family to transition into permanent housing.

Client last permanent address was located at 3034 Albany Crest Bronx, NY from Feb/2010 to April/2015. Family was evicted from the apartment. Client currently residency is 976 Tinton Ave. Unit Bronx NY. Family members are Jaythan Lopez (son 10y); Thalic Andrews (daughter 8y); Harold Andrews (son 7y); Angelina Ramirez (daughter 3y); Alekseia Ramirez (daughter 4months). Client stated that she does not have any children in foster care or a family member moving in with her at any time.

Client PA case is currently sanction and family is only eligible for Food Stamps for the amount of $218. Client is not eligible for Cash Assistance. Family does not have any other income. HoH Ms. Torres is unemployed due to daycare issues.

Torres stated that she is having a difficult time to find an apartment. HS De La Torre informed client about apartments in Staten Island; Ms. Torres stated that she does not want to move to Staten Island she does not know anyone over there and most of her family lives in the Bronx. HS asked client what about Project Reconnect or Family Reunification, Ms. Torres stated that her family is going through a rough time at the moment. Client may be eligible for CITY FEPS, however PA case is sanctioned. Family is not eligible for LINC, COME HOME NYC or EOSD due to lack of income. Clit applied for NYCHA back in 2007; Ms. Torres was advised to come back and reapply. HS asked client if, she needs assistance with anything and client stated “NO”. Family only option to transition to permanent housing is Section 8. Ms. Torres stated that she does not have any other option at this
