The patient I chose for this case study is a 98-year-old female, who lives next door to her son and daughter in law. She is a widow of six years and has been retired for over twenty years now. Her house is a one-story rancher, that has been made handicap accessible for getting in and out of the house. She was able to live independently, with her son available for help whenever she needed it. The patient wears glasses due to poor vison, wears hearing aids, and is right hand dominate. The patient’s prior level of function was independent level for bed mobility and transfer, and modified independence for level surfaces and stairs. The patient was able to ascend and descend 12 steps using bilateral hand rail support. She uses a rolling walker for ambulation and was able to ambulate house hold distances. All these prior levels of functions were gathered by the patient, family/caregiver, and medical records.
History of Present Illness/ Mechanism of Injury
The patient was hospitalized for pneumonia and recurrent/relapsing Clostridium Difficile Colitis. The patient was also present with hypertension, altered mental status, dehydration, fever, and hyponatremia.
Past Medical History/ Comorbidities The patient’s medical history is recurrent Clostridium Difficile Colitis,…show more content… Morphine impacted the patient’s ability to perform, due to the patient feeling foggy and tired. We would have to check with the nurses for a good time to see the patient between their medication to prevent this from happening, but it didn’t work in our favor all the time. A few times we would put the patient on 3L of oxygen because the patient’s oxygen levels would get low during
