Case Management Although, the Cicero community does not have a local hospital within the area, they do have clinics; which provide a limited amount of service. However, residents from Cicero use MacNeal Hospital in Berwyn as their primary hospital. The community health is presently being monitored by MacNeal Hospital by providing screening to measure health markers like their glucose levels, cholesterol levels and blood pressure. The MacNeal hospital works in collaboration with speaker’s Bureau on a great amount of health topics to promote primary, secondary and tertiary preventions. They also provide fairs to educate and promote the health of the community by teaching them about proper care as well as, measures that can help increase the quality of life. When it comes to the clinics that are located within the Cicero community a local organization called “Corazon Community Service” collaborates with the clinics to help improve the quality of life among the children and their families while offering a culturally sensitive environment. This measure is appropriate in relation to the issue that the community is currently facing. However, further implementations need to be address in order to make a change. In order to solve the health issues that the community is currently facing, we must reach out to the residents as well as, agencies to provide continuity of care in the community. The first step is to educate the residents about the current health issues that they are facing and why it is important for them to be aware. A barrier that can be identified is the resources that are available to the community. A main concern is the food desert that they encounter which leads to obesity and
