Client presents as an 83-year-old, Mexican-American female. Estella Luna lost her husband, Roger, two-years ago after a bout of lung cancer. Estella was her husband’s primary caregiver, and although it is reported that she and her husband had a joy filled relationship, the client struggled with caring for him, both emotionally and physically. Estella is still overcome with grief from the loss of her husband. Client has resided in the same home for the past 48-years. She has a dog and two cats, which are said to be her companions and motivating factors. It has been reported that the client has a few health problems, arthritis of the hip and back and circulation problems associated with late onset diabetes, but she has been managing them fairly well. Client takes several medications to manage the listed health concerns. Additionally, it appears that the client also suffers from dementia. Although it is uncertain where the client is at in the progression of dementia, because she resides alone, her unusual wondering around her neighborhood at atypical times, her reluctance to let her son, Michael, into her home, and her inability to identify him as her son, are symptoms that lean towards a dementia …show more content…
The intervention plan should consider what should occur if a goal is not attainable or is no longer attainable. Thus, it should consist of steps and plans to address what should happen next to answer “if” and “when.” For example, if it is determined that the client can no longer safely live on her own, how and where will she be cared for? It is crucial that plans are in place for the present and future needs of the client, so there is no gap in treatment or care, because of disagreement.
