During my shift I had the opportunity to communicate with a respiratory therapist and a member of the dialysis team. I meet my goal by asked questions related to team collaboration and listening to various team members share about responsibilities they each had in providing patient care. My patient had sepsis and was sedated with Propofol. I was in the room as the respiratory therapist came in to do suctioning and oral care. I inquired how frequently respiratory therapist comes in to the patient’s room to preforms care. The respiratory therapist explained that she comes in the preform care on patients at least every four hours. I also observed my patient received her second dialysis treatment. The nurse administering dialysis treatment informed me that the rate of blood being removed and filtered should be gradually increased within the first few treatments to see how the patient tolerates treatment. Patient receiving dialysis have to have overnight treatment or day time treatment that is three days a week. I identified my strength as being present during various treatments and interventions during care for my patient. A weakness that I identified was that I did not specifically identify the events leading up to my patient’s hospitalization. I read in the chart that she had a history of uncontrolled diabetes and heavy drinking. Those factors likely lead up to
