An incident report is never recorded in the patient chart and should never be copied. The incident report collects the details of the incident so that the risk management team where the information is entered into the system. The health care facility will then track the data along with other incidences that may have occurred and be able to determine if there are any trends they should be aware of. After review of the information the risk management team may decide that corrective action needs to be taken to prevent the same or similar incident from occurring again.
The only information recorded in the patient chart would be limited to the care given to the patient. With no mention of an incident will be mentioned/documented (Shaw, 2015, 229).
