On 02/02/2016 Ms. Eileen Palmer came to the SSO to meet with CM Mr. Best for an assessment. HS De La Torre took this opportunity to speak to client and develop a Housing Plan to explore housing opportunities available for the family. The following was discussed during meeting with HS.

Ms. Eileen Palmer informed HS that last permanent address was located at 230 Lott Ave. Brooklyn NY from 2008 to 2012. Family was evicted from this address; Ms. Palmer did not disclose the reason of eviction. At the moment, Ms. Palmer is residing at 1466 Grand Concourse Unit 2D Bronx, NY; admission date 1/13/2016. Family composition consists of Eileen Palmer (29y, self); Majesty Palmer (11y, son foster care); Moneesha Davis (7y, son foster care) Ms. Palmer informed HS that she came from Prospect Shelter, but stayed there for one week only because her two children were in foster care and Ms. Palmer is fighting through court to get her children back. Lastly, Ms. Palmer stated that no one will move in with the family if permanent housing is available. Ms. Palmer did not inform HS about any criminal records or programs that she is enrolled at the present or past time.

Ms. Palmer reported to be employed and earns about $1300 monthly. Also, Ms. Palmer PA case is fully active and she is eligible for FS

Palmer reported to have a LINC voucher for the amount of $1268 for one member while she was at the other facility. HS explained BNC does not have access to the voucher and she will have to update her PA case in order to try to obtain a renew voucher with the correct family composition, hoping to get a two bedroom voucher for the amount of $1515. Furthermore, Ms. Palmer is eligible for the CITYFEPS program due to eviction record. HS and CM has referred client to Bronxworks for a screening. HS will update the information as soon as Ms. Palmer provides us with one. Lastly, HS explain client about LINC VI, EOSD and COME HOME NYC programs, however client is not eligible for any of
