The Hmong community in general possesses a high level of distrust for traditional healthcare; its practices and practitioners. In an effort to eliminate the barriers between the community and healthcare system; this document will identify a program structure that will begin to address and over time reduce the barriers in order to assure collaborative efforts between the community and the healthcare system. The logic model will be the framework utilized to design the program in the Riverbend City. Kettner, Moroney, & Martin, 2017 state that the model operates on the following elements: inputs, process, outputs, outcomes, and impacts. Paul, 1993 asserts that the logic model is a theory of change that provides a snapshot of your proposed initiative. When applied effectively it provides a clear statement of the activities elicit change and affirms your projected results for the community and its people. However, the program developer must be mindful to reassess and identify when to review the progress and adjust accordingly. Kneale, Thomas, & Harris, 2015, state that logic models can be used as a tool for evaluation because the model offers a strategic point of reference. Furthermore, this will contribute substantial interactions between the healthcare provider and the Hmong patient to be more collaborative. Subsequently this will produce healthier community relations. Patients who are satisfied and feel that the provider respects their cultural views and preferences are more likely to be satisfied. Moreover, this will also enhance the likelihood of them reporting better health status, and lead to more consistent follow-up with the health care providers (Laws, & Chilton, 2013). In an effort to assess the strengths, limitations, and satisfaction of the staff and the clients; the program developer will use the qualitative method of research. The program will employ social surveys, and utilize Hmong community leaders to facilitate focus groups (Kettner, Moroney, & Martin, 2017).
