With over 15 years of case management experience, through the State of Wisconsin and UMOS, I am able to troubleshoot difficult cases with multiple active programs, through case simulations, yearly recertification and trainings, research, known policy and procedures, process help, and knowledge in IM programs including: Food Share, Child Care, Badger Care, Medicaid, and Caretaker Supplement. I also gather information through data exchanges, KIDS, and ECF. I am capable of troubleshooting and budget screens. Also, I retrieve information from Forward Health, CSAW, SAVE, and Opps Memos. I exercise a good judgment to manually determine eligibility of a client. Through my case management training, with the State of Wisconsin and UMOS, I have gained knowledge in reviewing/auditing per knowledge of the IM programs and applying policy and procedure. I am also able to provide info on FSET, W2, Marketplace and community partners, provide general agency info such as agency address, work hours, phone/fax numbers and web site address. (Premiums) individuals required to pay premiums are children in families with income over 200 % FPL; parents, stepparents, and caretaker relatives with income over 100% FPL, self employed parents, stepparents and caretaker relatives above 200% FPL. Premiums for adults are based on a percentage of the household income. Premium payments are due by the 10th of each month. (Benefit delivery) will consist of healthcare eligibility beginning the 1st day of the month determined eligible. FS benefits will be issued per the 8th digit of your social security number; each program is determined per income
