If I was the nurse in this position, I would try to speak with the client to deescalate his behaviors. I would follow the principles of mental health care listed in our text on page 80 box 8-1. I would start with exploring the patient’s behaviors and emotions. Asking questions, that is geared toward his behavior, for example, “I would tell the client making threats is inappropriate, and not allowed or Can you tell me why you are threatening other clients.” If the client’s behaviors continue to escalate I would contact the physician for a restraint and seclusion order.
