1. Decreased cardiac output r/t altered contractility AEB edema, fatigue, dyspnea, variations in BP readings, crackles, decreased ejection fraction, anxiety and restlessness. Schedule rest periods after meals and activities as well as provide a restful environment q shift. Monitor for chest pain or discomfort hourly.
2. Excess fluid volume r/t compromised regulatory mechanism AEB anxiety, blood pressure changes, dyspnea, edema, restlessness, weight gain, increased pulmonary congestion and orthopnea. Teach the patient to restrict sodium in the diet, weigh daily, take diuretics in morning, and monitor blood pressure daily q4h.
3. Activity intolerance r/t imbalance between oxygen supply/demand and sedentary lifestyle AEB abnormal blood pressure response to activity, abnormal heart rate response to activity, exertional discomfort, exertional dyspnea and verbal report of fatigue. Collaborate with physical therapy for gradually increasing activity q4h
4. Fatigue r/t anxiety, depression, poor physical condition, sleep deprivation and negative life events AEB disinterest in surroundings, drowsy, feelings of guilt for not keeping up with responsibilities, inability to restore energy even after sleep, increase in physical complaints, increase in rest requirements, lack of energy, lethargic, tired, and verbalization of an unremitting lack of energy. Teach patient to strategies to conserve energy such as sitting instead of standing during shower, and storing items at waist level q4h.
5. Imbalanced nutrition: Les than body requirements r/t psychological factors AEB aversion to eating, lack of interest in food, and reported food intake less than RDA. Monitor for signs of
