Per your request, enclosed please find a copy of Mr. Cordeiro’s “Participant Monthly History” form, indicating that the Fund office paid contributions to maintain Mr. Cordeiro’s family health benefits commencing July 31, 2008 for the previous month. Payments continued until late January, 2009, when Mr. Cordeiro returned to work. As we have already discussed, the total amount of payments for hours contributed by the Fund is $4,980.63, based on a total of 650 hours at $766.25 per hour.

Fund records indicate that Mr. Cordeiro received cash payments of $293.85 per week commencing October 5, 2008 (paid November 4, 2008) and continuing through January 17, 2009.
