Goal: Anger
Description: Naheem stated, “I want to improve my my anger and not get upset so quickly.”” For the next 12 months Jaheem will be able to reduce his anger outburst from 4-5x weekly to 0-1 times a week.
Obj: Over the next 360 days, Naheem will identify his triggers and and identify/ learn 3- 5 coping skills to assist with reducing his anger outburst.
Int: Therapist will explore with Naheem feelings and thoughts regarding not his anger toward his teachers and authority
Consmer Met his/her goal this session: partially Behavior:
Stephen E. Murphy, MS,S/T met with Naheem for the purpose of an individual counseling at his home. Naheem was appropriately dressed wearing khaki slacks, a white polo styled shirt and sandals. Naheem is currently in services for his oppositional defiant behaviors and poor conduct. Since last session there have been no reports by his mother that he has not behaved poorly or displayed any oppositional behavior.

S/T engaged Naheem about his feelings about his acquiring his ID and starting his GED program. S/T attempted to build rapport with client by allowing client to express his thoughts and feelings by utilizing active listening and CBT techniques with unconditional positive regard, genuineness and empathic responses. S/T provided information about Oppositional Defiant Disorder and benefits of being open to outside experiences. S/T presented Naheem with examples of how oppositional defiant disorder is received by others and can prohibit opportunities for him. S/T encouraged Naheem to open up about his feelings about his diagnosed disorder and share those
