You observe the following: Nurse A approaches Nurse B for an independent second verification prior to administering insulin to a patient. In the middle of the nurses station Nurse A shows Nurse B the insulin vial and the syringe with the insulin already drawn up. Nurse B looks at the vial and the syringe and then provides Nurse A with her/his verification code to enter into the electronic documentation system.

There are several errors I have observed in this picture. The Nurse A should have another nurse checks the verification in a less distracting environment and according to the hospital policy which is usually at the pyxis or the medication room. Nurse B is unaware

It is important to be honest while not judging or coming across them negatively as many nurses are unaware of the policy such as new graduates or probably did not receive the proper training. I would definitely offer to demonstrate the proper medication administration by verifying the five rights which are, the right medication, right patient, right dosage, right route, and the right time. I would also offer to go with Nurse A to verify the medication in the medication room. It is important to mention to them at the appropriate time the rationale behind certain procedures and protocols and the consequences of breaking them or finding shortcuts. Lastly, I would address my concern with my manager hoping to share the proper medication administration policy through huddle or education class.

Both nurses are violating patient safety by not performing the proper way of medication administration. Nurses must avoid workarounds and shortcuts. The should the obey the rules under every circumstance that will promote a safe environment to their patients.
