On 1/9/18, PCP was contacted by Paradise Health nurses and informed of constipation and incontinence issues with Barbara. A daily stool softener and PRN were prescribed to assist and issue was resolved after a few days. An order for Depends was given and group home staff contacted Home Care Delivery service for supplies. They were delivered in a few days.
On 2/16/19, Barbara visited PCP due to constipation and dehydration concerns and follow up to ER visit on 2/14/18. Dr. Avram discussed issues with poor PO intake and options to consider like feeding tube or hospice care. PCP contacted Barbara's brother to have POA resolved and discuss care options for Barbara. It was determined Barbara have a hospice consultation at group home next day.
