Scenario A

I would make an assessment of Fatma’s pain, to determine the severity, frequency, duration, what makes the pain worse, when makes the pain less, how the pain affects her daily life and what her perceptions are about her pain.

Fatma should not take Ibuprofen without food due to the risk of GI upset and bleeding. I would explain to Fatma the risk of taking Ibuprofen on an empty stomach. Depending on the severity and frequency of her pain, I would recommend that Fatma takes her medication close to 6 AM with food before she starts her fasting, and at 8 PM when she breaks her fasting. I would also talk to Fatma about other possible options such pain creams, rubs and patches to apply to her back as needed between 6 AM and 8 PM. If Fatma prefers to take her oral medication during her fasting time frame, I would recommend that she talks to her Imam about what her options are if she wants to use an oral pain medication for her back injury during her fasting. If this is possible, I could talk to Fatma about recommending a different oral analgesic that can be taken on an empty stomach.

I would recommend finding out why the patient has not been compliant with long-term medications. I would also recommend that the prescriber explains to the patient the potential harm that could result if she takes warfarin while taking ginkgo. The prescriber could tell her that the warfarin and the gingko do make the blood thin, but the warfarin does a better job at making the blood thin and its effectiveness can be evaluated and dose adjusted to meet her needs. I would ask the practitioner if he or she would want me to take a look at other anticoagulants that may not have serious interactions with
