Sierra Regional Center is a State agency that provides support and services to individuals with intellectual disabilities and/or developmental disabilities. Some of the services may include: service coordination, respite, supported living arrangements, jobs and day training, clinical services and self-directed programs. B. INTAKE APPOINTMENT

The majority of intake appointments are completed at Sierra Regional Center however the Intake Coordinator will accommodate the applicant and complete the intake at the individual’s home, hospitals, homeless shelters, NNAMHS, Jan Evans etc.

The applicant or legal guardian must complete the intake application and bring this to the intake appointment in addition to substantiating documentation such as information related to residency, financial eligibility and qualifying condition information. Psychological evaluations and testing including measure of adaptive functioning is required. Records of these assessments completed in the developmental period as well as more current assessments are needed to assist with determining eligibility.

1. During the Intake appointment the Intake Coordinator follows the following process:

a. Review the application for completeness and assist the applicant with completing the application.

All documentation and assessments will be photocopied for the chart.
e. Applicants will be informed of service options through Developmental Services and pertinent community resources and how to initiate application for these benefits or services.
f. Applicants are notified of the financial eligibility determination requirements.
g. The Intake Coordinator will complete an Initial Social Assessment.
h. The Intake Coordinator will assistance with in completing an application for Medicaid or Social Security if appropriate for the individual.
i. The Intake Coordinator will obtain supporting financial eligibility documentation to determine access to services
j. The Intake Coordinator will enter the applicant’s information into the Harmony database, obtain a consumer number and enter an application date.
k. All documentation is organized into sections in preparation for medical chart.
l. The Intake Coordinator will coordinate collection of assessment materials necessary to determine eligibility and/coordinate new assessments necessary for eligibility determination.
