STRENGTH-BASED MODEL

The strength based approach is a concept that firmly adheres to the belief that people or groups of people have existing competencies. It is an empowering approach that focuses on the individuals capabilities rather than their disabilities. People are generally seen as able to learn new skills, involved in discovery, have their own resources and can use the resources to address their own concerns.

The case manager organises services so that it is cost efficient and relevant to the client’s needs. The case manager provides continues monitoring and adds or deducts services according to the client’s changing needs or abilities.

Case Management Model: Assessment

In the case management model, the case manager assesses the needs of the client. It first starts with a functional assessment. The person living with dementia is evaluated of his or her ADL skills, cognition, ability to remain safe, and socialise. The case manager figures out individual needs and aspirations in life. The assessment is based on the existing abilities of the person he or she then compiles a completed appraisal and proceed with making a coordinated rehabilitation plan.
