2-Low carbohydrate diets have been shown to be effective but are not without risk due to the limited consumption of fruits, vegetables and grains. If a client wishes to follow this type of diet, it is your responsibility to make sure that they follow it in the safest manner possible.

4-Assessment indicates client currently has low self efficacy (unemployed and lives with parents) and conflict with family value system (perception that mother prepares fattening food and insists client "clean her plate"). Client engages in negative self talk and appears to be depressed. Though she came to counseling on her own accord, she appears to be in the pre-contemplation stage of change related to making changes to promote weight loss including food and physical activity behaviors. May also have unrealistic nutrition-related goals - believes weight loss will lead to employment. Eating in response to boredom is also an issue. NC 3.3 Overweight/Obesity related to physical inactivity, excess energy intake, food and nutrition related knowledge deficit and not ready for lifestyle change as evidenced by high BMI, increased adiposity, and reports of large amounts of sedentary activities, inability to apply nutrition-related recommendations, history of familial obesity and overconsumption of high-fat and/or calorie density food or beverage, verbalized perception of lack of support from family and that they sabotage her efforts, pre-contemplation stage of change and low self-efficacy.
