The study was conducted with the aim of measuring the Health Care Transition readiness using the Transition Readiness Assessment Questionnaire (TRAQ) survey tool among 79 youths between the ages of 16 to 25 years and 25 parents who were receiving care from an adolescent or pediatric specific clinic. The analysis was done using logistic regression controlling for age and gender and the result showed that 70% of the youths and 67% of the parents believed in their ability of self-managed care with higher TRAQ score signifying more likelihood in the belief of one’s ability of self-managed care.
