An evaluation plan is an instrument used to thoroughly evaluate a program (Zint, n.d.). Evaluation also assesses a program’s success, or limitations, and guide the decision-making process (Zint, n.d.). This program was designed to study the effectiveness and practicality of implementing a structured education and training program to improve health outcomes in those with type 2 diabetes over the course of six months. The program objectives include the regular performance of physical exercise, improved glycemic control, and the reduced risk of associated disease complications.
Data was collected via tools that were designed to assess patient knowledge and self-efficacy, improved performance of regular exercise, daily fasting glucose levels, metabolic laboratory values, as well as staff knowledge prior to and following plan implementation (see Appendices). Participants will be inclusive of all type 2 diabetics within the current organization, in order to realize the program’s feasibility on a grand scale. In addition, the process by which the program would be implemented was assessed and deemed suitable for the purpose of the study.
