Knowledge evidence questions

1) I would get information regarding a service user’s specific communication and language needs either from their Person Centered Plan that is stored in the staff office, or I would speak with my house manager and other staff members who would be able to advise me. If the service user was able to let me know I would ask them. Or I could read the service users daily notes.

5) If a service user could not understand what I was trying to communicate to them I would try to write it down if they are able to read, or use macaton or a picture board if they are able to understand that way.

7) You should ask the person concerned if they have the mental capacity to understand what you are asking, or you would speak with your manager. Records that are held about the service user are private and confidential and are on a need to know basis only if there for example was suspected abuse would you need to access their records.
8) I would speak to the senior on duty to inform them and then I would record it in the service user’s daily notes. I would also write a note in the staff communication book to make others aware that there had been a problem.
