Here are a few things I noticed that need fixing from reviewing the spreadsheets from CTBD. -Vital spreadsheet: It appears that the patient’s vital signs correlate with the dates from their lab results. For some of our patients, their vitals are taken a day or two prior to their labs draws and those are the vitals being reported on their CRF. Our Epi patients have multiple vitals documented in CRIS during their week admission, but I need vitals pulled from a specific date. I’m assuming this can be possible?
Epi-01: on 09/23/2018 vitals present and not matching CRIS, but patient had vitals done on 09/22/2018.
Epi-01: on 11/20/2018 vitals present and not accurate. Vitals were done on 11/19/2018, 11/21/2014 & 11/23/2014. I want the vitals

Epi-02: line 19 for 07/31/2014: body weight, body surface, body height and heart rate aren’t in CRIS. They may not be this patient’s information.
Epi-02: need vitals from 04/06/2014, not 04/07/14.
Epi-02: 04/07/14 has body weight, body surface & body height charted, but they were performed on 04/06/14 and the data is not correct. The heart rate is 81 instead of 83.

-CRIS lab spreadsheet: Missing a few data required for protocol. Initially, I thought adjusting the date range was not the solution to this issue. However, from reviewing I think increasing the range will fix this issue. Majority of our Epi patients are here for a week. Labs are done throughout the entire stay, especially at the beginning and end of their visit. Can we add additional 3-4 days at the beginning and end of the date entered for labs collection?
Epi-01: During the blinded baseline (04/03-07/14), CTDB pulled over labs on 04/04/2014 & 04/07/2014, but missing labs drawn on 04/03/2014.
Epi-02: During the blinded ARM 2 month 8 (10/06-10/14), CTDB pulled over labs on 10/06/2014, 10/07/2014 & 10/08/2014, but missing labs drawn on 10/10/2014.
Epi-04: During the blinded baseline (03/17-21/14), CTDB pulled over labs on 03/17/14, 03/18/2014, but missing labs drawn on 03/21/2014.
