On 7/12/17 Joseph Gomez, Wellness Coach (WC) and Mr. Dunlap, Asst. Director made a visit to Mrs. Annette James (Tenant) at her home for the purpose of introduction and following up on her mental health, medical wellness and to address any issues she may be experiencing. When WC arrived Tenant was eating and WC was greeted at the door by tenants’ home attendant. Tenant was quite cordial towards WC. Tenant was dressed appropriately and was groomed. Tenant appeared to be mentally stable.
SYMPTOM MANAGEMENT
WC asked Tenant how he was doing and she responded that she was fine. Tenant reports that she has a Home Health Aide services five days a week.
Tenant stated that she is doing well and is getting all her entitlements and money from
