The patient arrived early to her counseling session. This writer introduces herself as the patient assigned counselor moving forward, at which the patient was pleased. This writer reviewed the patient's UDS result and Clinical Director email to address the ongoing use of illicit benzos. This writer then questioned the patient about her recent UDS result as the patient self-admits to using illicit benzo every other day, it's either the xanax and/or klonopin. The patient recent use was yesterday, one mgs of xanax and klonopin (1 pill of each). This writer rendered a patient engagement plan and advises the patient that the illicit use of benzo is unacceptable for treatment and questioned the patient about her plans to discontinue her use. According to the patient, she has a schedule appointment with ICRC on 6/5/2017 to obtained prescription for her benzo use.

The patient discuss her past heavily of when she started to started to use illicitly, it started with her prescription to oxycodone and then to heroin when her prescriber discontinue her use. According to the patient, she is no longer using heroin as her drug of choice for nearly 6 months. Furthermore, the patient shared with this writer that she is diagnosed with anxiety, depression, and PTSD. The

The living arrangment is toxic, whereas the patient detail that her husband father is verbally abusive towards her. In addition, the patient husband's father is dignosed with dementria and may have diabetes but not taking any treatment for his diabetes to avoid losing his CDL. Suggestion were discuss with the patient to address the living situtation. The patient and her husband plan to leave the residence once their finances are stable but the concern is about the patient's husband father health; however, the verbal abuse is not healthy for the patient depression and anxiety as it tends to heighten to the point that it is has
