14 YO P.S was admitted to the facility on 8/11/17 following a suicide attempt after taking 16mg of Xanax and blacking out. His medical diagnosis is major depression disorder recurrent severe with no psychotic factors, polysubstance abuse and nicotine dependence. Depression is known to be caused by environmental and biological factors. Drug abuse is common among people who have depression, drugs can temporary numb or lift moods while working on the CNS as a depressant or stimulant.
This patient has a hx of SI and depression. His father reported “he has had these thoughts for years” and his first suicide attempt was in 2nd grade. He admits to self-medicating to feel euphoric. He admits to using serval drugs and buying them off “the black market”
