Joanne is unemployed but has income from disability pension. Joanne lives with her mom and two siblings in a very small rural community about four miles out of Birchville. The family consists of her mother who is 65 years old and her three siblings. The oldest sibling is Marvin who is married and lives and works in Steeltown, 300 miles away. The second sibling is Lorraine, age 32 years old, who is single. Lorraine lives with Joanne Cassel and she works as a cook at a local retirement home. Joanne's youngest sibling, Frances, is age 29. Frances is a single parent, living at home with a six-month-old baby. Joanne's father, John Cassell, is deceased. According to the file, Joanne told one of the nurses that her father was alcoholic and passed away due to alcoholism.

The client has some medical problems, such as kidney failure because she did not follow dietary and insulin regimen, which caused a loss of vision. The most important thing that the client needs is to get dialysis treatment three times a week. She needs secure transportation to her appointments.
