Telephone contact made to the FOB. Two patient verifer used to confirm name and DOB. Due to prior conversation with FOP Sgt Brooks sat in on the conversation. Per Lt Wu, FOP advised that his son has a stress injury on the L5 but no stress fracture. He has strain an Schmorl's nodules, which are vertical protrusions of disc fluid have created nodules on the L5/S1 vertebrae. Therefore, FOP was advised that and appt with his PCM is needed. Patient states that he would prefer to f/u with Lt Wu because she saw his son in the past and reviewed the MRI results. Appt scheduled. See below. FOB was informed to arrived 15mins early prior to appt. FOP agreed and verbalized understanding to the POC.
