IDENTIFYING DATA: Patient is a 26-year-old White male serving 11 years of Burglary, Grand Theft, Fraud Credit Card, Grand Theft Firearm, Burglary Armed, Trafficking in Stolen Property, Fake ID to a Pawn Broker, and Altering ID of a Firearm. He entered DOC on 01/18/13 form Brevard County. His TRD is 05/20/2023. This is the patient’s first incarceration. II REASON FOR ADMISSION: Patient was transferred to CSU due to suicidal ideations stemming from frustration with a perceived lack of mental health support. While in CSU all intervention use to decrease patient’s self-injurious behavior and to increase his interpersonal coping skills, patient has not been successful. As a result, it was recommended for a higher level of care in which MHTF to better manage his current behavior. III CHRONOLOGY OF TREATMENT & PATIENT'S PROGRESS:
Specific Problems Treated: Diagnosis: F32.9 Pertinent depressive D/O, Unspecified ( Provisional), F43.1 PTDS(Provisional), F62.3 Borderline personality D/O. ISP goals are #101 Abuse to Self, #308 Discharge Planning and Readiness Skills/Active, and #113 Depression Interventions Used: Psychiatric assessment, nursing assessments, medication compliance monitoring, individual counseling/case management, Rec, & MH Group activities. Patient is meeting his treatment goals for problems areas. Patient has verbalized continuing his current psychotropic medications & counseling on a TCU setting as well as in the community. Patient is eating 3 meals per day, sleeping 7-8 hours, showering when offered, and attending out of cell activities, such as individual sessions with his case manager & psychiatrist. Patient has verbalized a clear understanding of his behaviors and has indicated the capacity to make decisions regarding his care. Currently, he’s a level 2 risk
