Omari is a 4th grade boy with black hair and brown eyes. He is an extrovert with a very outgoing personality. He is always willing to speak with teachers and students when classes are over. Although he is one of the shortest kids compared to his peers, he has a passion for playing sports such as soccer and baseball. His favorite sports team is the New York Yankees and enjoys watching NBA basketball. Omari has a slight intellectual disability. He is in a comprehensive work environment but requires an aid in each of his classes to help him with the different assignments and tasks that are given. His verbal communication is good, but at times he exhibits a slurred speech. At time his patience runs low and his upset when he is unable to understand a task or an assignment. Omari also has a slight hearing impairment causing to wear hearing aides. He wears glasses for his vision impairment. When performing certain physical activities, Omari exhibits limited muscle strength in his upper and lower body. This may cause him to have problems with keeping up with the rest of his peers.

MOTOR SKILL PRESENT LEVEL OF PERFORMANCE ASSESSMENT
Motor Skill: Kicking
Test, Description and Rubric and Criteria, Results, Goal, and Activities
· Test: TGMD II – Object Control – Kicking
· Description: Omari will run and kick an 8-10 inch foam ball using proper form. He will be given two trials to achieve a maximum score of 4.
· Rubric and Criteria:
- 4: Excellent form and technique (demonstrated 4 out of the 4 criteria)
- 3: Good form and technique (demonstrated 3 out of 4 criteria)
- 2: Average form and technique (demonstrated 1-2 out of 4 criteria)
- 1: Below average form and technique (demonstrated 0 out of 4 criteria)
- 0: Did not attempt
· Results: Omari received a 1 (below average form and technique) in both trials. Omari was able to make contact with the ball on both trials but showed incorrect form when approaching and making contact with the ball.
· Object Control Goal: By the end of the school year, Omari will be able to improve his kicking technique by completing 3 of the 4 kicking criteria during both trials.
· Activities
1. Kicking a foam ball off of a very low tee and taking a step back from the ball each time he has successfully demonstrated proper form and technique from that distance.
2. Kicking foam ball at large targets from a short distance (e.g. 5 feet away). As Omari begins to master shooting at these targets, have him kick at targets that are farther away and are slightly smaller. In order to hit the targets, he will learn that he needs to use proper kicking form to increase the chances of succeeding.

· Test: TGMD II – Locomotor - Leap
· Description: Omari will run and leap over a beanbag that is 10 feet away from the starting point. He will be given two trials to achieve a maximum score of 4.
· Rubric and Criteria:
- 4: Excellent form and technique (demonstrated 3 out 3 criteria)
- 3: Good form and technique (2 out of 3 criteria)
- 2: Average form and technique (demonstrated 1 out of 3 criteria)
- 1: Below average form and technique (demonstrated none of the criteria)

· Results: Omari received a 1 on his first trial and 2 on his second trial. On his first trial, he demonstrated more of a jump on his landing. On his second trial, he was able to land on his opposite foot on his landing but still demonstrated poor technique in his arm motion and lift off phase.
