Assessment begins with the first contact with a client and is based on the establishment of understanding and relationships. Questions are asked to obtain facts, clarify perceptions, validate observations, interpret meanings and confirm information. The use of open-ended and closed question can provide a basis for both the assessment and relationship with the client.
