PRESENTING CONCERN (Reason for seeking services and related history):
He presented with anxiety, depression, and relational problems with peers. He reported increased depressed mood, anxiety, isolation, and anhedonia in the past few weeks. He reported that some days he stays in the bed all day. He noted that his appetite diminished with an increased anxiety. He indicated that his anxiety and depression gradually increased since the mid-Winter quarter.

He came to the appointment on time. He dressed appropriate to the weather. He was Ox4. He made eye contact throughout the session. He was engaging. He appeared to be anxious in the beginning of the session. He seemed to relax after finding out that we could conduct the intake in Japanese. He smiled and laughed more toward the end of the session. His speech was congruent. He had good observations about his experiences and symptoms; however, his insight about them were limited.
