My present position with Bellin Health’s organization is directly within ambulatory care. Bellin Health is piloting a refill team to allow patients to focus on their direct concerns with their provider face to face then providers able to engage with each patient through a newly establish care team known as Team Base Care. Team Base Care is built with a MD /NP/PA, 1-2 LPN’s/CMA’s depending on the number of patient’s seen per day. LPN’s/CMA’s responsibilities include reviewing medications, addressing refills, assisting providers as well as scribing in the room for providers. As many are familiar medication barcoding was implemented to prevent medication errors, improve patient care through safety, and improve real time documentation accuracy. Medication bar coding is highly depended on technology through medication scanners and documentation software (example Epic). Not only did I practice medication barcode scanning, but additionally firmly believe in acknowledging nursing six right of medication administration through direct practice.
Through my experience as a float pool nurse, I joined the patient bedside report committee. Bedside report allows for a clear concise visual for the next shift nurse, and the current nurse to review patient information (Diagnosis, Allergies, Plan of treatment, IV Lines, mobility, ect) with engagement among each patient. Patients benefit in many ways such as warm hand off-introduction, clear communication, opportunity to answer questions/fears/concerns, built trusting relationship, establish a pain control plan, and provide individualized quality
