Electronic health record (EHR) system have been adopted into the medical profession to help medical staff work more productively and to achieve higher levels of quality on the job. EHR transform healthcare experiences for clinicians, office staff as well as patients by changing the documentation and filing of patient’s health records. Prior to the implementation of EHR, illegible handwritten documents negatively affected the quality of healthcare service because documents were misplaced, attached to wrong patient files, and there were many unreadable printing. Furthermore, charts in the exam rooms were cumbersome for nurses and physicians while flipping through sheets of paper to review patient charts.

Although there are still chance of human errors while inputting data, digitizing the process drastically decreases the margin of error. In addition, EHR supports the reporting of clinical quality measures (CQMs) because it helps the government measure and track the quality of healthcare services and measures a wide variety of data usage that are associated with a provider’s ability to deliver high-quality care, or related to long-term goals for healthcare quality. For example, it tracks smoking status and exchange key clinical data with physicians to predetermine goals and key performance indicators for particular treatment plans of patients.
