Biographical Data Nicholas Johnson is a 27 year old Caucasian male. He was born March 24, 1990 in Willingboro, NJ. He is married and currently resides at 314 New Jersey Rd. Browns Mills, NJ, 08015. He is phone number is 609-651-6889. He is a third year medical student at Rowan School of Osteopathic Medicine with the intention of pursing Emergency Medicine for his residency. English is his primary language and he is agnostic.

He is aware he is underweight and states he’s been at this weight for the “last 10 years or so”. He states he has tried gaining weight through exercise and protein shakes with no success. He denies fatigue, malaise, fever, chills, and night sweats. Nicholas claims he has had no change in his skin, freckles, or moles. He states he is “rarely outside” and wears sunscreen when he plans to be outside for an extended period of time. He denies using a tanning bed. RN encouraged patient to continue to not use a tanning bed and educated patient of risk of skin cancer from use of tanning bed (Zhang et. al, 2012). Nicholas denies bleeding gums with brushing or flossing. He denies alterations in taste and dental appliances. Nicholas states he brushed his teeth “for about a minute” and flosses each night. RN educated patient on brushing for at least two minutes and the importance of good oral hygiene and its impact on overall health. RN informed patient that recent research suggest “a new link between gum disease and heart disease” (“Brushing teeth reduces risk of heart disease”, 2008, p. 462) and that “when the immune system reacts to oral infection, it also attacks host proteins, causing arterial disease” (“Brushing teeth reduces risk of heart disease”, 2008 p. 463). Nicholas states he is aware and will “try to extend my brushing time”. Nicholas denies limitation if range of motion of the neck, neck pain and swelling. Nicholas denies axillary tenderness, lumps, or rashes. Nicholas denies history of lung disease and chest pain with or without activity. He denies snoring.
