Therapeutic Communication In A Patient's Case

Patient was ready and willing to learn. Identified by her eye contact and by verbally responding to my questions. Patient is in need of therapeutic communication after refusing to leave hospital after being discharged.

Patient was upset and crying she was hunched over with her hands on her abdomen. She had a really loud low tone voice and stated that she was in pain and frustrated. Patient was yelling and made remarks like “I’m not ready to leave” when I entered with the wheel chair. She also stated that she was “over this” repeatedly.

The barriers of communication identified was that she was crying at the time of communication. She was also in obvious distress and pain. The interventions that I put in play is making her in ice bag for her back to take home she preferred cold rather than hot. I handed her a tissue box and identified if she had medication on board which she did about 10 minutes ago.

Patient is a frequent flyer who was in the ED the night before. After, assessment and lab values were analyzed it was determined that the patient did show any signs of infection or other complications. Patient was given pain medicine and the physician gave her a prescription for oxycodone with discharge information. As I came in to discharge her she refused and started crying on her bed. She stated that she was not well enough to go home. I came in with the wheel chair and she stated I’m not ready to go yet I’m waiting for my ride. She than stated that she had enough of this and could not continue.

Analysis/Diagnosis: After, speaking with her and analyzing her non-verbal cues I came to the conclusion that she was experiencing fear to go home. This was confirmed after she stated I am scared to go home and being alone.

Plan: Patient will use the available Chaplin at the end of our
