Purpose statement. This critical thinking paper is developed to highlight the importance of medication administration error and review medication administration-related accident prevention. Method This paper will approach the situation by describing an incident, by performing an analysis, and by offering a solution using data, theory, and the nursing context. This paper includes my perceptions of nursing leadership and evolving perceptions of the healthcare system while I observed an incident. The analysis includes identifying the terms and root cause. Situation Incident: I was working in one of the Philadelphia’s hospital emergency departments (ED) during the night shift. (Härkänen et al., 2020) The administration has to follow policy and not exceed the recommended nurse-to-patient ratio. Another step in medication administration safety in the ED, there should be a clear and efficient reporting system that should address the steps where these errors are being made frequently. Reporting medication errors provides valuable data for quality improvement initiatives. Analyzing the root causes of errors helps identify areas for improvement in medication administration processes, and protocols. To promote reporting, hospitals must train nurses to identify clinical errors and self-report them (Ghobadian et al, 2021). Providing comprehensive education and training on medication safety protocols, error reporting procedures, and the importance of reporting errors can help increase nurses' awareness and understanding of their role in error reporting. Leininger, a pioneer in transcultural nursing, believed that culture profoundly impacts individuals' health beliefs, practices, and responses to illness and treatment. Her theory highlights the need for culturally competent care to effectively meet the diverse needs of patients from different cultural backgrounds. It focuses on the fact that different cultures have different caring behaviors and different health and illness values, beliefs, and patterns of behaviors (Leininger et al.). Philosophy The nursing theory I have selected and identified most with is Madeleine Leininger’s Transcultural Theory. I am a woman of West Indian descent, born in Gujarat. As part of my heritage and personality, I love meeting new people from diverse cultures. Cultural competence is a key aspect of nursing care. I believe in embracing, studying, and analyzing the health and care practices, beliefs, and values that contribute to nursing while honoring their backgrounds and cultural values. Leininger’s nursing theory embraces the knowledge and details of a patient’s family, their communities, and the institutions in which a diverse healthcare system exists.
