On Friday, October 23, 2015, Officer Jamael Smith heard Inmate Ingram Cole R/I 234070 make a threat to do bodily harm to Inmate Brain Vaughn R/I 203536 via the Inmate Telephone System. On Monday, October 26, 2015, the Shelby County Sheriff’s Office (SCSO) was notified. Inmate Cole was placed on administrative segregation.

On Monday, October 26, 2015, the information concerning the threat made on Inmate Vaughn was passed to SCSO Deputy Mack. Deputy Mack and Sergeant Robert Elliot heard the telephone call made by Inmate Cole. On Monday, October 26, 2015, Officer Smith intercepted another telephone call made by Inmate Cole that alleges misconduct by Officer Demetrius Haley. The telephone call suggested that Inmate Haley had informed Inmate Cole that the writer had spoken to Inmate Vaughn. On Wednesday, October 28, 2015, Officer Haley gave a tape recorded video interview. Officer Haley denied all allegations that he informed Inmate Haley that OPS has called for Inmate Vaughn and that he did not called Inmate Haley from the dorm and speak with him.

As of today’s date there will not be an investigation by SCSO into the telephone call threat made by Inmate Cole. Inmate Vaughn was questioned by the writer and he refused protective custody and did not want to be moved from D-Bldg. Inmate Cole was moved from the building and placed on Administrative Segregation until further notice. A classification override was submitted and approved on Thursday, October 29, 2015 to raise Inmate Cole’s level from a six to a seven and house him in E-Bldg. The telephone calls coming from D-Bldg will be monitored to determine if any more threats are
