I had a productive day at Holy Spirit. I learned how to become more organized with planning my day for my patient. I also learned how to receive report and give report to my primary nurse. I was able to come up with three nursing diagnoses, interventions, and outcome to go accompany my diagnosis. I realized that it can be quite difficult figuring out what nursing diagnosis is appropriate for my patient. I loved discussing the concept map with my clinical instructor during the debriefing session instead of actually turning the paper in by a certain time. It helped having group discussions with my colleagues and my clinical instructor about the medical diagnosis and the pathophysiology of my patients. Health assessment lab was productive this week. I learned how to chart more specifics assessments in SIM Chart. I learned that health and wellness of the patients are important. It is imperative for me to properly assess my patient, so that I can report my abnormal finding to the primary nurse. This week my patient had a sacral decubitus ulcer and I noticed that she was not eating much for breakfast. I educated her about the importance of nutrition to help promote the healing of her ulcer. I encouraged her to eat more and she did. That was a great feeling to see that my education helped my patient. I need to focus on my health and wellness and eat more nutritious foods as well. Last week, we talked about the cardiovascular disease as well as peripheral artery disease and I realized that African Americans are more at risk of developing these diseases. Being as though, I am at risk I need to exercise at least 30 minutes a day and reduce saturated and trans fat from my diet and eat more fruits and
