(ii) Once you have completed the Hazard ID checklist, fill in the Action Plan for up to six hazards you identified. (iii) Conduct a meeting with your trainer and other colleagues and discuss any items that you have on your Hazard ID template or Action Plan. Record the minutes of the meeting using the template provided.(iv) Finally, interview your workplace supervisor / trainer, and use the OHS Assessment Sheet to fill in the details.
