S: Child was alert but lethargic some of the time
O: the following activities were performed:
-Sensory processing activities: Swinging in the hammock swing in linear motion with medium swings for 6 minutes followed by 1-10 counts for transitioning to another activity. Wilbarger's brushing protocol (providing deep pressure, tactile input, and joint compression), weight-bearing activity, free play activity, kneeling activity, sitting activity and ball exercises to increase postural control, fine and gross motor skills, UE strength, bilateral coordination, and activity tolerance to facilitate therapeutic activities.
-Weight-bearing activity: the child was placed in the wheelbarrow position to increase UE strength and proprioceptive inputs.

-Ball exercises: Using thera-ball to enhanced UE strength and balance.
-Behavioral modification techniques: Positive reinforcement using therapeutic use of self to motivates the child to participates in the OT directed activities.

A: Child was alert but lethargic some of the time. He exhibited good visual tracking of the toys and was noted to maintain 2 minutes with both hands placed on the guitar without removing his hands. During the Wilbarger’s bushing protocol, he maintained sitting position for up to 3 minutes without sliding out of the chair. He was observed to tap on the ball for few minutes while he was in the hammock swing. He maintained 4 minutes of kneeling while playing with musical toys. However, he still exhibits poor postural control which inhibits him from reaching toys or other objects.
